NEW COACH REQUIREMENTS

Welcome to Yreka Union High School District.  Required documentation is due back to the District Office prior to coaching.

Required Documentation:

• Personnel Employment Information Form 
• Live Scan – BY APPOINTMENT ONLY - @ SCOE  
• Valid TB Skin Test (Public Health Dpt. or personal Doctor – starting at $20.00)
• Verification of completion of Fundamentals of Coaching Certificate ($50.00 reimbursable for volunteers)

• Complete Course

Reimbursement Instructions (Volunteers Only):

• Complete Reimbursement Form
• Attached the Fundamentals of Coaching Certificate and Receipt

• Verification of completion of CNCSN (for Concussion Training/Certification)

• DMV Pull Program Authorization Form to Release Driving Record Form for 25 years of age and older (Required to drive students)
